Whole Systems Integrated Care (WSIC) is a national pioneer programme for integrated care. Whole Systems working breaks down the barriers between different professions and services. It will help overcome fragmentation between health, mental health, social care and the third sector. Whole Systems is designed to effectively support people with complex conditions – in Brent we are now moving to WSIC Phase 2, focusing first on people aged 65 and over with one or more long term conditions.

 

In 2015/16 we will trial an approach where care is delivered to this population group by a multidisciplinary team through a partnership of providers. Staff will build new relationships with each other and with their patients, service users, carers and the third sector. Operations and ways of working will be developed and aligned.
